The Board of Directors of the Putnam County Hospice and Palliative Care Association (PCHPCA) announces its annual report to the community.  PCHPCA seeks to enhance the quality of life for the terminally ill, their loved ones, and caregivers, by educating the Putnam County community about the benefits of hospice, palliative care, and advance care planning.  

In spite of the continued effects of the COVID-19 pandemic, PCHPCA accomplished a great deal in 2021.  A new three-year 2022-2024 strategic plan was completed.  More activities were conducted in 2021 for National Healthcare Decisions Day, World Hospice and Palliative Care Day, and National Hospice and Palliative Care Month.  The Free Vital Medical Information File Program was initiated and new partnerships have been forged with first responders, law enforcement, and the medical community.  Materials for 750 files were purchased, and more than 500 Files have been distributed.  PCHPCA applied for an ARPA grant to be able to produce enough Vital Medical Information Files for all senior citizens and seriously ill persons in Putnam County, and will hopefully receive approval.  Social media engagement was aggressively pursued, and its effectiveness in advancing our mission continues to be evaluated.  Remote meetings have continued to be productive for PCHPCA.
