TWO WAYS TO DONATE:
The Putnam County Community Foundation accepts donations for the Association. Putnam County Hospice and Palliative Care Association is a nonprofit public charity, and your gifts are tax-deductible to the fullest extent allowed by law. There are two ways to donate to the Association:
Donations via check and mail:
- Make check payable to The Putnam County Community Foundation.
- Write PCHPCA FUND on the memo line of the check.
- If you wish to make your donation in honor or memory of someone, indicate this either on the memo line or on a separate enclosure.

Donations online using credit card:
The Putnam County Community Foundation accepts online donations for the Association. Clicking below will take you to the Putnam County Community Foundation’s website to make an online donation. Instructions are provided below.

- Choose a donation amount.
- Designate / choose the Putnam County Hospice and Palliative Care Association to receive the donation.
- Indicate whether this is a one-time or recurring gift.
- If you wish to make your donation in honor or memory of someone, click the box under tribute gift and provide the name of person(s).
- Fill in the name and address of the person making the donation.
- Fill in the credit card information.
- Click donate.
