“We chose the blue color so that the folder would be easily visible in a person’s file drawer for easy reference,” said Elaine Peck, Director.  Peck stated that adults of all ages and health conditions should have legal advance healthcare directives and conversations with loved ones about their wishes for healthcare in the event of an unexpected accident or serious illness.  “And just like estate and retirement plans, your healthcare wishes should be reviewed and updated regularly, especially at important life milestones, such as significant birthdays, marriage, birth of a child, divorce, and retirement.”

If you or your organization is interested in partnering with the Putnam County Hospice and Palliative Care Association to help distribute “My Advance Directives” folders, please contact PCHPCA at contact.us@pchpca.org or 765.301.7614.

The Putnam County Hospice and Palliative Care Association (PCHPCA) seeks to enhance the quality of life for the terminally ill, their loved ones, and caregivers, by educating the Putnam County community about the benefits of hospice, palliative care, and advance care planning.   PCHPCA is a nonprofit, public charity, and operates with the help of donations, financial support from the Putnam County Community Foundation, and fiscal sponsorship by the Putnam County Hospital.
